Job Description:

Essential Duties and Responsibilities:

  • Oversees the development, revision, and the implementation of specific program policies and procedures, and insures that they are in compliance with S.U.P.R., FGC, and with the hospitals. 

  • Ensures GPRAs associated with the Warm-Hand Off are completed per its specifications.

  • Builds and maintains relationships with healthcare, mental health, and substance use organizations to share program services, coordinate care, and support the needs of their populations. 
  • Manages counseling and care coordination services, multi-disciplinary treatment plan development and collaborates to integrate S.U.D. and primary care services at Cook County Health satellite clinics. 
  • Approves all schedules insuring adequate and continuous coverage for all programming at site. 
  • Drafts employee probationary and annual performance reviews,discusses these prepared documents with the VP makes the necessary revisions, and delivers the review with each employee he/she directly supervises in a timely manner. 
  • In conjunction with FGC’s Human Resources Department, makes employment (hiring, promotion, termination, re-assignments) recommendations 
  • Onboards and trains new staff, ensuring a comprehensive understanding of role responsibilities, workflows, and organizational policies. 
  • Ensures that each person who presents for services is assessed and referred to the appropriate level of care.  If inpatient services are indicated, ensures that their clinical condition meets ‘medical necessity’ requirements, and that these assessments are effectively and promptly communicated with managed-care entities to gain authorization for treatment. 
  • Ensures that each patient served has appropriate discharge and linkage plans, and that these plans are communicated effectively to the next level of care. 
  • Oversees the preparation and submission of all required weekly and/or monthly statistical productivity reports on all program aspects to FGC leadership and hospital administrative staff. 

 

Location: Thorek Hospital Andersonville

Schedule: Monday - Friday (7:00a - 3:30p); Will need to be available during 2nd shift and weekends to address any staff needs.

Start Date: ASAP

Pay Rate: $50,000 - $55,000 annually

Experience and Skills:

Education and Experience: High School Diploma or GED equivalent. Bachelor’s degree preferred. 2+ years of behavioral healthcare management experience in a hospital or community based setting, or medical detox unit program required.  Working knowledge of program development concepts, ASAM clinical guidelines,  

Certifications, Licenses, Registrations: IAODAPCA Certified Alcohol and Other Drugs Counselor (CADC) required .  Clearance from the CFS 708-C Background Check for Non-Licensed Contract Staff, and the Criminal Background Check for Healthcare Workers.
