Family Guidance Centers, Inc. is seeking a Case Manager to increase engagement in care for racial and ethnic medically underserved individuals with substance use disorders (SUDs) who are at risk for or with HIV. As part of CSAT’s Minority AIDS Initiative (MAI) program, the Case Manager will collaborate with staff at The University of Illinois Health-Mile Square Health Center (MSHC) to provide education, case management, and referral/linkage to Pre-Exposure Prophylaxis (PrEP) services for individuals who screen negative for HIV but are at increased risk of exposure to HIV. The Case Manager will also engage individuals with or at risk for HIV in evidence-based substance use disorder treatment and recovery support services. Essential Duties and Responsibilities: · Conducts HIV screenings, provides HIV pre/post-rest counseling, completes MAT intake interviews and assessments and connects patients to the HIV service provider partners (UIH-Mile Square) for HIV treatment and PrEP (pre-exposure prophylaxis) services. · Works closely with the syringe exchange referral source (COIP) and other harm reduction organizations to provide support for individuals identified at high risk for Haemophilus influenzae type b who also have opioid use disorders (OUD). · Uses evidence-based techniques including Motivational Interviewing to support patients in identifying and creating goals and developing recovery plans with the skills, strengths, supports and resources to aid them in achieving those goals. · Takes a holistic approach to care to address a patient’s willingness and motivation for change; addressing self-confidence and other emotional triggers that affect change and support the patient’s ability to embrace and sustain positive change related to Substance Use Disorder and HIV. · Follows up with patients to ensure they remain engaged in Substance Use Disorder treatment and recovery support services as well as HIV treatment and prevention services. Schedule: Monday - Friday 6:00 AM - 2:30 PM; based on agency needs Start Date: ASAP Work Locations: 310 W. Chicago Ave., Chicago ( with occasional travel to 2630 S.
