Essential Duties: · Coordinate patient care with Methodist Hospital, Nurse Practitioner, FGC through a collaborative approach. · Provides PRS services to a full caseload, achieving a minimum of 20 billable hours weekly (can include telehealth modality). · Complete and maintain accurate records or reports regarding patients’ histories and progress, services provided, or other required information. · Act as liaison with the MCO designated transportation companies. · Administer toxicology testing to new and current patients. Must have at least one year of recovery from substance use, mental health or co-occurring substance use and mental health challenges. Certificates, Licenses, Registrations: Certified by the Illinois Certification Board (ICB, Inc.) as a Certified Recovery Support Specialist (CRSS) or as a Certified Peer Recovery Specialist (CRPS) or be certified within one year of employment date.
