We’re seeking an experienced Program Manager/Counselor to lead clinical operations for our new Opioid Treatment Program Partnership (OTPP) at UI Health Mile Square Health Center in Rockford.

This grant-funded initiative, supported by Illinois RCCA opioid settlement funds, expands our successful Chicago partnership with UI Health Mile Square Health Center Chicago, to bring methadone treatment and integrated primary care to one of Illinois’ most impacted regions for opioid overdose and substance use disorder.

You’ll help build a collaborative, multidisciplinary team that provides life-saving, whole-person care—integrating addiction treatment, medical services, and behavioral health under one roof.

 

Essential Duties:

Program Management and Clinical Supervision

• Provide clinical direction, support, and supervision to staff assigned to the Rockford site.

• Deliver ongoing performance feedback, conduct 30-day competencies, 90-day and annual evaluations, and issue commendations or disciplinary actions as appropriate.

• Coordinate integrated patient care with the UI Health primary care team, including physicians, nurse practitioners, nursing staff, and peer recovery specialists.

• Present patients at weekly multidisciplinary staffing meetings, reporting treatment progress, barriers, and recommendations.

• Participate in and facilitate team meetings, staffings, and case consultations.

• Support administrators in maintaining program compliance, quality assurance, and productivity standards.

• Manage patient transitions to higher or lower levels of care, coordinating with external providers when necessary.

• Oversee toxicology testing protocols—ensuring supplies are ordered, screenings occur as scheduled, and results are documented and addressed within required timeframes.

• Identify opportunities for procedural improvements and recommend policy updates as needed.

• Promote a positive, customer-focused culture that emphasizes teamwork, accountability, and quality patient outcomes.

Counseling and Clinical Services

• Provide direct individual, group, and family counseling, as well as case management and patient education services.

• Conduct intake assessments, complete treatment plans, and coordinate discharge and aftercare planning.

• Utilize evidence-based and motivational approaches to engage patients in recovery and promote abstinence from illicit substances.

• Monitor and document patient progress in alignment with individualized treatment goals and measurable objectives.

• Modify treatment plans as clinically indicated by changes in patient status.

 

Start Date: December 1st, 2025

Work Location: 1221 State St., (U of I Mile Square Health Center), Rockford, IL 61102

Schedule: Monday - Friday (6:00a - 2:30p) and rotating Saturdays (6:00a - 9:00a)

Pay Rate: $55,000 - $60,000

Experience and Skills:

Education and/or Experience: High School Diploma / GED required; Two years of management experience in Behavioral Health, preferably in an outpatient setting is preferred.

Certificates, Licenses, Registrations: IAODAPCA Certified Alcohol and Other Drugs Counselor (CADC) required, or able to obtain within allotted time of hire date.
