Job Description:

                                            1st $500.00: after 30 days of employment

                                           2nd $500.00: after 6 months of employment

Essential Duties and Responsibilities: 

• Communicates and assists all patients with their transportation needs which include regular contact with their insurance company and constant tracking and updating of all standing orders.
• Acts as liaison with the MCO designated transportation companies.
• Facilitates group instruction at minimum 1 time weekly
• Processes all newly admitted patients from referring detox unit into level 1 OMT (including scanning all documents daily).
• Coordinates with detox staff via email from referring hospital with appropriate paperwork as required.
• Communicates with treatment team(s) and referring hospital units to ensure a smooth transition (Nursing staff, clinical team, and Tox department).
• Provides education on Naloxone to all newly admitted patients as well as ongoing education on the importance of why/how to administer and use it and document.
• Follows up with Audits/Arcs to ensure all appropriate paperwork is collected/scanned into SAMMS on a timely basis (including income verification, picture id, SSN and Verification of addiction).
• Reviews assessed case management needs as expressed by clients and as indicated on the client treatment plan.

Number of Position Openings: 1

Schedule: : Must work a minimum of 40 hours per week and available for Saturday, opening and closing (if needed).  Standard business hours are Monday–Friday 8:00 AM to 4:30 PM (can vary as based on needs of patients/peer/case manager’s schedule). Hours may vary based on program needs of agency and flexibility is required. 

Pay Rate: Up to $19.23 per hour; based on experience

Start Date: ASAP

 

Experience and Skills:

Education and/or Experience: Bachelor of Science or Bachelor of Arts degree in Social Work, Psychology, Counseling, or related field is preferred. CADC preferred. One year of experience in the field of addictions is desired. Work knowledge of chemical dependency, chemical abuse, chemical addiction, 12-step programs, principles of helping, and/or willingness to pursue training.

Certificates, Licenses, Registrations: Certified by the Illinois Certification Board (ICB, Inc.) as a Certified Recovery Support Specialist (CRSS) or as a Certified Peer Recovery Specialist (CPRS) or be certified within one year of employment date.
