****$2,000 SIGN ON BONUS****

1st $1,000.00: after 30 days of employment

2nd  $1,000.00: after 6 months of employment

Essential Duties:

· Provides direct supervision to all staff nurses allocated to FGC at Joliet, which include timekeeping approvals, commendations, disciplining, competencies and performance evaluations.

· Provides direct training, education and orientation to new staff allocated to FGC Joliet.

· Complete infection control data collection and quarterly/annual reporting.

· Processes patients from assessment to discharge for medication assisted treatment, drug free treatment programming and assess patient for appropriateness for assigned clinics.

· Communicates with the HR department to coordinate and interview nurses seeking employment at the FGC Joliet clinic.

· Participates in the agencies’ performance improvement, infection control and audit correction processes.

· Responsible for follow up with documentation for Pain Assessment and Nutritional Questionnaire patients.

· Available via company issued mobile phone to communicate with any appropriate FGC staff including nurses and managers after hours.

· Administration of Vivitrol injections. Processes patients within all Vivitrol revenue funding sources. Provides educational awareness to patients that Vivitrol is available as a third option in medication-assisted treatment.

· Follows up with the county jail medical team to discuss patient’s dosage and relays dose requests to a FGC physician.

Schedule: Monday - Friday (7:30 AM - 4:30 PM); may require some weekends

Pay Rate: $68,000 - $78,000

Education and/or Experience: Two years’ experience in hospital or in-patient setting preferable. Individual must possess good communication skills with basic nursing knowledge. Knowledge of substance abuse pathology preferred. Employee must have a strong knowledge of all applicable state and federal rules and regulations.

Certificates, Licenses, Registrations: R.N. or L.P.N. licensed in the State of Illinois and certified in CPR.
