Job Description:

Family Guidance Centers, Inc. (FGC), a not-for-profit behavioral healthcare organization, treats and prevents substance abuse, as well as an array of other related problems. Since 1969, FGC's comprehensive programming has educated and guided individuals toward their full potential as productive members within their own families and our society.

Essential Duties and Responsibilities:

Substance Abuse Counseling

  • Develop patient treatment plans and conducts continued stay review based on research, clinical experience, and patients’ histories - using SAMMS assessment on computer.
  • Provide patient intakes, treatment planning, case coordination, staffing, aftercare planning and discharge with individual and group therapy for a full spectrum of referral sources.
  • Assess individual’s degree of drug dependency
  • Coordinate counseling efforts with mental health professionals or other health professionals
  • Review and evaluate patients’ progress
  • Counsel patients individually or in group sessions
  • Plan or implement follow-up or aftercare programs for patients to be discharged from treatment programs.

Mental Health Therapist 

The Mental Health Therapist is a Qualified Mental Health Professional [QMHP] who has the education, skills and experience to conduct a clinical assessment, develop a plan of treatment and provide treatment for a person with mental illness, under the supervision of a Licensed Practitioner of the Healing Arts [LPHA].

  • Completion of an initial Mental Health Assessment for all mental health clients.
  • Construction and documentation of an individualized treatment plan (ITP) in conjunction with the client,
  • Provision of individual, group, couples/family counseling as driven by the needs identified in the assessment
  • Provision of mental health case management to facilitate linkage to and ongoing coordination and support for needed services and supports in patients’ community environment.
  • Documentation of an Admission Note summarizing the clients’ identifying information, presenting problem, history of present problem, provisional diagnosis, safety precautions, the medical necessity of treatment and treatment plan recommendations.

Experience and Skills:

Education and/or Experience: Master’s degree in Psychology, Social Work, Counseling, or a related discipline from an accredited institution preferred.

Certificates, Licenses, Registrations: State of Illinois licensure required (i.e. Licensed Professional Counselor, Licensed Clinical Professional Counselor, Licensed Social Worker or Licensed Clinical Social Worker). In addition to the above licensure, IAODAPCA Certified Alcohol and Other Drugs Counselor certification or proven experience with substance use treatment preferred.

Salary Range: $36,000 - $40,000 Annually
