Job Description:

Family Guidance Centers, Inc. (FGC), a not-for-profit behavioral healthcare organization, treats and prevents substance abuse, as well as an array of other related problems. Since 1969, FGC's comprehensive programming has educated and guided individuals toward their full potential as productive members within their own families and our society.

Essential Duties and Responsibilities: 

Job responsibilities include working as part of a multi-disciplinary team of the hospital staff and FGC staff responsible for assisting, monitoring and coordinating services for Medical Detox Patients.Provides clinical oversight to the Unit by conducting staff clinical trainings and assisting non-clinical staff by providing the necessary clinical supervision.

  • Assists the Unit Administrator in developing clinical curriculums for treatment groups and reviews and signs ASAM reviews and other relevant clinical documents.
  • Assists in the admission process for patients in consult with multi-disciplinary team.
  • Orients new patients to the Medical Detox Unit.
  • Assists in performing comprehensive psychosocial assessments on patients in order to determine appropriate level of care and discharging planning needs.
  • Meets with patients in individual, group, and/or family settings.  Counselors may assist patients with a variety of concerns, but the primary focus of the Medical Detox Unit is for patients to be bio-medically stabilized and to be referred to another level of care upon discharge.
  • Conducts informational/educational sessions providing appropriate materials to educate patients on addiction recovery and aftercare options.
  • Assists with patient continued stay review process with the established procedure to monitor the medical necessity and appropriateness of continued hospitalization.
  • Involves patients in the discharge planning process and respects and advocates for patient’s rights in all aspects of their care.  Reviews discharge information with patients.
  • Acts as coordinator for discharge needs of patients assigned to him/her.  Maintains current knowledge of resources available within the community, maintains supply of resource materials to be distributed to patients when needed.  Is able to network to obtain other resources for patients as needed.
  • Works collaboratively in formulating an appropriate and realistic discharge care plan, communicating with the patient.  Completes appropriate forms, and contracts all appropriate individuals to facilitate the transfer/linkage of patients to other community based agencies and services.  Ensures all referrals are linked through follow-up.
  • Documents in charts all relevant clinical information pertaining to patients based on assessment and education and discharge planning activity.

Schedule: Monday thru Friday 7:30 am - 4:00 pm; Depending on caseloads needs hours may change

Experience and Skills:

Education and/or Experience:  Bachelor’s degree in Psychology, Social Work, Counseling, or a related discipline from an accredited institution preferred. Master’s degree preferred, not required. 

Certificates, Licenses, Registrations:  IAODAPCA Certified Alcohol and Other Drugs Counselor required.  State of Illinois Licensed Clinical Professional Counselor, Licensed Professional Counselor, Licensed Social Worker or Licensed Clinical Social Worker preferred.

Salary Range: $32,000 - $35,000 annually
