Job Description:

The Substance Abuse Counselor Essential Duties and Responsibilities at the Joliet office include but are not limited to: Complete SAMMS intake assessments for new patients; Complete an individualized treatment plan for each patient on caseload, addressing all needs identified in the psychosocial assessment; Provide group and individual sessions to patients in accordance with their treatment plan problems, goals, and objectives; Properly document all services provided in individualized progress notes per DASA, Joint Commission, and FGC regulations, standards, and policies, summarizing the patient’s status and ongoing treatment needs; Review and evaluate patients’ progress and continued stay in treatment in relation to measurable goals and objectives described in treatment and care plans. Modify treatment plans to comply with changes in patient status; Plan and assist with the implementation of aftercare and continuing recovery plans once patients are discharged from treatment programs; Complete opening and closing data collection tools (GPRAs) for STR program patients, and send to the designated contact person at least weekly; Work with patient and other staff, when necessary, to deescalate and assist with patient emergencies and crisis situations; Coordinates care with legal, law enforcement, and treatment professionals, both within and outside the agency, to help ensure patient needs are met and increase positive treatment outcomes. This would include Will County representatives, Safe Passage program contacts, and others; Participate in any treatment team staffing.

Salary starting at $32,000 per year

Work Hours: Monday thru Friday 8:30 am - 5:00 pm Saturday rotations

Experience and Skills:

Education and/or Experience: Bachelor’s degree (or higher) and prior substance abuse treatment experience preferred. Will consider relevant internship experience in lieu of paid employment.

Certification, Licensure, Registrations: Licensure (LPC, LCPC, LSW, LCSW) or CADC certification preferred, and required within two years of starting work as a counselor. Clearances fromCFS 689 Child Abuse and Neglect Tracking System, and CFS 708-C Background Check for Non-Licensed Contract Staff.

Skills:

  • Properly administer and read alcohol analyzers.
  • Ability to use analytical software.
  • Ability to use calendar and scheduling software.
  • Experience using desktop computers, copier, and fax machine.
