Job Description:

Essential Duties and Responsibilities (at Glenwood building):

  • Develop patient treatment plans and conducts continued stay review based on research, clinical
  • Provide patient intakes, treatment planning, case coordination, staffing, aftercare planning and discharge with individual and group therapy for a full spectrum of OMT referrals.
  • Complete and maintain accurate records or reports regarding patients’ histories and progress, services provided, or other required information.
  • Interview patients, review records and confer with other professionals to evaluate individual’s mental and physical condition and to determine their suitability for participation in a specific program.
  • Coordinate counseling efforts with DUI evaluators and mental health professionals, or other health professionals, such as doctors, nurses, or social workers.
  • Review and evaluate patient’s progress in relation to measurable goals described in treatment and care plans.
  • Modify treatment plans to comply with changes in patient status.
  • Assists in troubleshooting of day to day operations as an advocate for patients to resolve emergency problems in crisis situations.
  • Counsel patients individually or in group sessions, to assist in overcoming dependencies, adjusting to life or making changes.
  • Assists AVP with recommendations for staff hires and discharges.
  • Train new support staff, clinical employees, and interns.
  • Provide prompt performance evaluations, clinical competency reviews and help staff define learning and training needs as well as the development of future goals.
  • Conduct regular clinical staffings and team meetings and document.
  • Provide direct clinical service to a partial caseload, achieving 10 billable hours weekly.
  • Coordinate and approve staff schedules, time off requests and maintain Paylocity timekeeping records
  • Supervise Glenwood Office Manager and PM Office/Care Manager, ensure that documentation for FGC accounting, such as daily cash reconciliation, other accounts payable information, accounts receivable information, and service production verification (e.g. reporting of toxicology production, IL.DHS.SUPR openings, etc.) is accurate and timely.
  • Provide crisis intervention as needed using appropriate de-escalation and crisis intervention techniques.
  • Provide community relations interacting as appropriate with other providers and community members (e.g. DCFS, local Police Dept., local businesses, treatment facilities, health and mental health service providers, etc.)
  • Ability to travel to multiple program sites; is flexible with time and travel; and has a current Illinois Driver’s License and automobile insurance coverage. Must provide copies of driver’s license and proof of insurance to Human Resources.

Number of Position Openings: 1

Schedule Hours: Monday thru Friday 6:30am - 3:00pm - with some flexibility to meet agency needs and day to day operations

Saturdays Rotation: 7:30am - 11:30am

Experience and Skills:

Education and/or Experience: Bachelor’s degree and supervisory experience preferred. Criminal justice, medication assisted treatment, leadership, and supervision skills are necessary to ensure staff and program development.


Certificates, Licenses and Registrations: State of Illinois Certified Drug and Alcohol Counselor, State of Illinois Licensed Clinical Professional Counselor, Licensed Professional Counselor, Licensed Social Worker or Licensed Clinical Social Worker required. Candidates must have one of the following; CADC, LCPC, LPC, PhD, MD, LSW, or LCSW certification/ licensure. Clearance from CFS 689 Child Abuse and Neglect Tracking System.
