Job Description:

Essential Duties and Responsibilities: Provides intake assessments for all levels of care and assists with the coordination of admission when appropriate under supervision of the Program Manager. Supports the Mission, Values, and Vision of Family Guidance Centers and the facility.

  • Answers daily inquiries of patients and public in person or via telephone regarding admitting procedures, agency regulations and services; when necessary, refers inquiries to appropriate person or department elsewhere in the agency; provides directions for finding other areas within the agency.
  • Completes financial assessments as needed.
  • Screens treatment inquiries and triages for assessment/admission based on need per ASAM Patient Placement Criteria.
  • Completes intake assessments and makes provisional diagnoses based on DSM-IV TR under supervision of the Program Manager.
  • Completes intake assessments and makes recommendations for placement to all levels of care based on ASAM PPC.
  • Makes appropriate referrals to other SUD treatment programs when appropriate treatment services are not available at agency.
  • Makes appropriate referrals to ancillary agencies when special needs are assessed.
  • Handle patient referrals of an urgent nature, both internal and external.
  • Provides scheduled intake assessments.
  • Act as a liaison with community referral sources.
  • Maintains patient confidentiality at all times in accordance with 42 CFR and HIPAA regulations.

Standard Requirements:

  • Possesses a working knowledge of FGC applicable Standard Operating Procedures (SOP).
  • Knowledgeable of patient rights and ensures an atmosphere which allows for the privacy, dignity, and well-being of all patients in a safe, secure environment.
  • Attend in-service and other training events to continue education and expand knowledge to improve the quality of patient services.
  • Ability to follow written and oral instructions and procedures.

Schedule: 3rd Shift Monday - Friday 10:00pm - 6:30am with weekend rotation

Experience and Skills:

Education and/or Experience: Bachelor’s degree preferred. Certification or commiserate experience will be considered.

Certificates, Licenses, Registrations: Certified Assessment and Referral Specialist (CARS) is required at minimum within 2 years from date of hire. CADC preferred.

Skills:

  • Properly administer and read alcohol analyzers.
  • Ability to use analytical software.
  • Ability to use calendar and scheduling software.
  • Experience using desktop computers, copier, and fax machine.
  • Experience using electronic mail software.
