Job Description:

Essential Duties and Responsibilities:

  • Develops patient treatment plans and conducts continued stay review based on research, clinical experience, and patients’ histories - using SAMMS assessment on computer. Provide patient intakes, treatment planning, case coordination, staffing, aftercare planning and discharge with individual and group therapy for a full spectrum of referral sources.
  • Provides direct clinical service to caseload, (full caseload requires a minimum of 20 billable hours weekly).
  • Completes and maintains accurate records or reports regarding patients’ histories and progress, services provided, or other required information.
  • Assesses individual’s degree of drug dependency by collecting and analyzing drug screening samples.
  • Interviews patients, review records and confer with other professionals to evaluate individual’s mental health and physical condition and to determine their suitability for participation in a specific program.
  • Reviews and evaluates patients’ progress in relation to measurable goals described in treatment and care plans.
  • Modifies treatment plans to comply with changes in patient status.
  • Intervenes as an advocate for patients or patients to resolve emergency problems in crisis situations.
  • Counsels patients individually, or in group sessions, to assist in overcoming dependencies, adjusting to life, or making changes.
  • Confers with family members or others close to patients to keep them informed of treatment planning and progress (written authorization by patient must be obtained - refer to Federal Confidentiality Regulations for substance abuse treatment).
  • Counsels family members to assist them in understanding, dealing with, and supporting patients (written authorization by patient must be obtained - refer to Federal Confidentiality Regulations for substance abuse treatment).
  • Provides patients or family members with information about addiction issues and about available services or programs, making appropriate referrals when necessary.
  • Plans or implements follow-up or aftercare programs for patients to be discharged from treatment programs.
  • Follows progress of discharged patients to determine effectiveness of treatments.
  • Supervises or direct other workers providing services to patients.
  • Performs tasks which are supportive in nature to the essential functions of the job, but may be altered or redesigned depending upon individual circumstances.

  • Assists in directing others workers providing client services.

  • Possesses a working knowledge of FGC applicable Standard Operating Procedures (SOP).

  • Knowledgeable of patient rights and ensures an atmosphere which allows for the privacy, dignity, and well-being of all patients in a safe, secure environment.

  • Attends in-service and other training events to continue education and expand knowledge to improve the quality of patient services

  • Other duties as assigned

Location: 25000 Euclid Avenue Suite 406, Euclid, OH 44117

Schedule: Monday-Friday 6:00am-2:30pm, some occasional Saturdays 7am-11am, depending on agency needs.

Experience and Skills:

A minimum of an Associate’s degree is required. A Bachelor’s degree in Psychology, Social Work, counseling, behavioral science or related field is preferred. One year of experience working with individuals with substance abuse/chemical dependency disorders required. Qualified individuals must be able to provide case management, crisis intervention, and counseling, as treatment for substance abuse. CDCAII, LCDC II or LCDC III license and in good standing with the state board is required.
