Job Description:

Essential Duties and Responsibilities:

  • Develops patient treatment plans and conducts continued stay review based on assessed needs and progress.
  • Provides patient intakes and comprehensive assessments, treatment planning, case management and referrals, staffing, aftercare planning and discharge planning.
  • Counsels patients individually or in group sessions, to assist in overcoming dependencies, adjusting to life, or making changes.
  • Completes and maintains accurate records or reports regarding patients’ histories and progress, services provided, or other required information.
  • Interviews patients, reviews records and confers with other professionals to evaluate individual’s mental and physical condition and to determine their suitability for participation in a specific program.
  • Coordinates counseling efforts with mental health professionals or other health professionals, such as doctors, nurses, or social workers.
  • Reviews and evaluates patient’s progress in relation to measurable goals described in treatment and plans.
  • Modifies treatment plans to comply with changes in patient status.
  • Intervenes as an advocate for patients to resolve emergency problems in potential crisis situations.
  • Provides prompt performance evaluations, clinical competency reviews, and help staff define learning and training needs as well as the development of future goals.
  • Conducts regular, scheduled, clinical supervision meetings with each counselor and document.

  • Provides direct clinical service to a partial caseload, achieving minimum 10 billable hours weekly.
  • Coordinates and approves staff schedules, time off requests and maintains Paylocity timekeeping records.
  • Ensures FGC Team participation in all fire and safety codes/drills.
  • Ensures concise written reporting of any incidents and forwards to FGC risk management officer, as apart of risk management.
  • Knows and disseminates FGC policies and procedures, periodically reviewing key SOP areas with staff.
  • Contributes to new ideas and problem solving communicating with administrative supervisor.
  • Actively contributes to a culture of high level communication by clearly modeling active communication to staff and soliciting information and feedback from staff and sharing with the management team.
  • Supervising the office manager, ensures that documentation for FGC accounting, such as daily cash reconciliation, other accounts payable information, accounts receivable information, and service production verification (e.g. reporting of toxicology production, patient treatment openings, etc.) is accurate and timely.
  • Provides crisis intervention as needed using appropriate de-escalation and crisis intervention techniques.
  • Provides community relations interacting as appropriate with other providers and community members(e.g. ODJFS, local Police Dept., county probation departments, local businesses, treatment facilities, health and mental health service providers, etc. ) and maintains active linkage agreements.
  • Serves as an effective role model for written and verbal communication.
  • Performs tasks that are supportive in nature on the essential functions of the job, but may be altered or redesigned, depending upon individual circumstances.


Schedule: Monday thru Friday7:30am-4:00pm. Saturday rotation, 7:00am-11:00am. Schedule may vary based on clinic and agency needs, to include 6:00am start time.

Experience and Skills:

Education and/or Experience: Master’s degree in a behavioral healthcare field with licensure is required. Experience with substance use treatment required. Leadership and supervision skills are necessary to ensure staff and program development.


Certificates, Licenses and Registrations: State of Ohio Licensed Independent Chemical Dependency Counselor-CS, (LICDC_CS), Licensed Independent Social Worker Supervisor (LISW-S) or Licensed Professional Clinical Counselor Supervisor (LPCC-S) required. Must have experience with substance use treatment and above active credentials required to supervise substance abuse and mental health treatment services.
