Job Description:

Family Guidance Centers, Inc. (FGC), a not-for-profit behavioral healthcare organization, treats and prevents substance abuse, as well as an array of other related problems. FGC is currently looking for a self-motivated, innovative Care Coordinator for its Springfield location. Family Guidance Centers Inc. offers opportunities for individuals with an interested in behavioral health. Essential Duties and Responsibilities:

  • Assessment: Completes a comprehensive assessment of care management needs (i.e. necessary service elements) and documents in SAMMS for use in initial and ongoing treatment planning. This includes identification of resources needed such as; substance use recovery supports, medical, mental health, dental, housing, employment, childcare, transportation, financial assistance with budgeting or application for benefits, etc.
  • Planning: Identifies and documents a plan with objectives and timelines for accessing and utilizing needed services (this information will be integrated into the patient’s treatment plan).
  • Engagement and Education: Ensure patients understand who the members of the care team are and what the guidelines, purpose, and expectations for treatment services are. Establish realistic treatment and recovery expectations with the patient and explain in a way the patient can understand goals, procedures, rules regarding conduct, treatment schedule, any cost of treatment, factors affecting continued stay and patient rights and responsibilities. Meet with patients on a periodic basis as needed to monitor and facilitate progress on care management goals using evidence based practice (e.g. MI, Behavioral Activation) to support positive change.
  • Coordination: Establish working knowledge of the service elements and procedures for patient access to community services. Determine when patients need assistance coordinating access to services versus self-referral and in accordance with confidentiality guidelines assist patients in contacting providers and securing service. Assist in coordinating transitions for aftercare and discharge planning.
  • Monitoring and evaluation: Monitor and document the outcome of referrals made confirming that patients accessed care and helping to evaluate the effectiveness of referrals made.
  • Outcome Tracking:Assist with tracking selected outcome measures such as; regular program service attendance and retention, progress on addressing reduction and elimination of substance use, engagement in supportive services including recommended healthcare for recovery and wellness maintenance, progress on employment and/or application for benefits, securing of safe housing, etc. Assist with agency tracking types and volume of referrals made and effectiveness for continued understanding of population needs, community resources and barriers to care.
  • Consultation: Effectively summarize patient needs and progress offering feedback that will assist the multidisciplinary team within and outside the organization in planning and modifying the treatment plan. Maintain professional, responsive, and collegial communication that respects the patient and all other care providers.
  • Supervision and Professional Development: Participate in supervision with assigned supervisor as well as appropriate continuing education activities related to care management and maintenance of relevant licensure.
  • Direct Service and Documentation: 50% of work time is spent in provision of billable service with timely documentation according to FGC and applicable state and federal guidelines.

Schedule: Monday thru Friday 8:00am-4:30pm with flexibility in schedule to accommodate agency needs, with Saturday Rotation

Experience and Skills:

Education and Experience: Minimum bachelor’s degree in psychology, social work or related field with relevant social service field internship or some related experience. Master’s Degree preferred but not required. Understanding of substance use disorders, mental disorders, and importance of well- coordinated care required. Basic knowledge of the local care service delivery system and skills in motivating patient behavior and coordinating direct care linkage required. Two years of experience working with substance use and/or mental health preferred.

Certificates, Licenses and Registrations: IAODAPCA Assessment and Referral Specialist required.  Specifically noted, CAR is required at minimum within 2 years from date of hire.  Additionally, Certified Alcohol and other Drugs Counselor CADC is preferred
