Job Description:

Family Guidance Centers, Inc. (FGC), a not-for-profit behavioral healthcare organization treats and prevents substance abuse, as well as an array of other related problems is currently seeking a Licensed Practical Nurse with some experience with drug treatment as well as clinical experience. Family Guidance Centers Inc. offers opportunities for individuals with an interest in behavioral health. Essential Duties and Responsibilities:

Mental Health Assessment

1. Prompt scheduling and completion of an initial Mental Health Assessment for all mental health clients. This Mental Health Assessment shall be reviewed and approved by an LPHA.

2. Completion of The Level Of Care Utilization System (LOCUS) functional assessment for all

mental health clients 18 years of age and older.

3. Completion of the Columbia Impairment Scale and Ohio Scales for Youth for all client’s between the age of 5 and 17 years.

4. Completion of the Devereux Early Childhood Assessment Clinical Form (DECA-C) for all children 0 through 5 years of age.

5. Documentation of an Admission Note summarizing the clients’ identifying information, presenting problem, history of present problem, provisional diagnosis, safety precautions, the medical necessity of treatment and treatment plan recommendations.

6. Re-assessment per Medicaid Title 132 guidelines for ongoing patients.

Treatment Planning

1. Construction and documentation of a timely individualized treatment plan (ITP) in conjunction with the client, and their guardian, in the event they have a court-appointed guardian or are a minor, addressing the clinical needs identified in the assessment process, coordination of care and discharge planning needs. This ITP shall be reviewed and approved by an LPHA.

2. Timely revision/modification of the ITP as the client moves through the treatment process.

Mental Health Therapy and Case Management

1. Provision of individual, group, couples/family counseling as driven by the needs identified in the assessment process and as indicated in the individualized treatment plan.

2. Timely documentation of therapy in progress notes summarizing for each session; goals and objectives addressed, clinical interventions, client response to interventions, and continued therapy plan.

3. Provision of mental health case management to facilitate linkage to and ongoing coordination and support for needed services and supports in patients’ community environment.

4. Crisis intervention when indicated including immediate coordination of indicated service given nature of crisis addressed.

5. Consistent engagement of clients including prompt follow up on attendance issues or barriers, on use of referrals made, etc.

6. Consistent collaboration with the multi-disciplinary team to support integrated care in regard to treatment process.


** The candidate must be able to spend at least 50% of work time providing direct billable services.

Experience and Skills:

Education and/or Experience: Master’s degree in Psychology, Social Work, Counseling, or a related discipline from an accredited institution preferred. The Mental Health Therapist is a Qualified Mental Health Professional [QMHP] who has the education, skills and experience to conduct a clinical assessment, develop a plan of treatment and provide treatment for a person with mental illness, under the supervision of a Licensed Practitioner of the Healing Arts [LPHA].

Certificates, Licenses, Registrations: State of Illinois licensure preferred not required (i.e. Licensed Professional Counselor, Licensed Clinical Professional Counselor, Licensed Social Worker or Licensed Clinical Social Worker). In addition to the above licensure, IAODAPCA Certified Alcohol and Other Drugs Counselor certification or proven experience with substance use treatment preferred. License eligible candidates with a master’s degree may be considered.
