Job Description:

Family Guidance Centers, Inc. (FGC), a not-for-profit behavioral healthcare organization treats and prevents substance abuse, as well as an array of other related problems is currently seeking a Mental Health Therapist Intern, currently enrolled in a Masters' in Counseling or Social Work Program.  Family Guidance Centers Inc. offers opportunities for individuals with an interest in behavioral health.

Essential Duties and Responsibilities: 

OMT Counseling:

  • Develops patient treatment plans and conducts continued stay review based on research, clinical experience, and patients’ histories - using SAMMS assessment on computer.  Provide patient intakes, treatment planning, case coordination, staffing, aftercare planning and discharge with individual and group therapy for a full spectrum of referral sources.
  • Completes and maintains accurate records or reports regarding patients’ histories and progress, services provided, or other required information. 
  • Assesses individual’s degree of drug dependency by collecting and analyzing drug screening samples.
  • Interviews patients, reviews records and confers with other professionals to evaluate individual’s mental and physical condition and to determine their suitability for participation in a specific program.
  • Coordinates counseling efforts with mental health professionals or other health professionals, such as doctors, nurses, or social workers.
  • Reviews and evaluate patients’ progress in relation to measurable goals described in treatment and care plans.
  • Modifies treatment plans to comply with changes in patient status.
  • Intervene as an advocate for patients or patients to resolve emergency problems in crisis situations.
  • Counsels patients individually or in group sessions, to assist in overcoming dependencies, adjusting to life, or making changes.
  • Confers with family members or others close to patients to keep them informed of treatment planning and progress (written authorization by patient must be obtained – refer to Federal Confidentiality Regulations for substance abuse treatment). 
  • Counsels family members to assist them in understanding, dealing with, and supporting patients (written authorization by patient must be obtained – refer to Federal Confidentiality Regulations for substance abuse treatment). 
  • Provides patients or family members with information about addiction issues and about available services or programs, making appropriate referrals when necessary. 
  • Plans and/or implements follow-up or aftercare programs for patients to be discharged from treatment programs. 
  • Follows progress of discharged patients to determine effectiveness of treatments.

     Mental Health Assessment

    1.      Completion of an initial Mental Health Assessment for all mental health clients. This Mental Health Assessment shall be reviewed and approved by an LPHA.

    2.      Completion of The Level Of Care Utilization System (LOCUS) functional assessment for all

                   mental health clients 18 years of age and older.

    3.      Completion of the Columbia Impairment Scale and Ohio Scales for Youth for all client’s between the age of 5 and 17 years.

    4.      Completion of the Devereux Early Childhood Assessment Clinical Form (DECA-C) for all children 0 through 5 years of age.

    5.      Documentation of an Admission Note summarizing the clients’ identifying information, presenting problem, history of present problem, provisional diagnosis, safety precautions, the medical necessity of treatment and treatment plan recommendations.

    6.      Re-assessment per Medicaid Title 132 guidelines for ongoing patients.

      Treatment Planning

    1.      Construction and documentation of an individualized treatment plan (ITP) in conjunction with the client, and their guardian, in the event they have a court-appointed guardian or are a minor, addressing the clinical needs identified in the assessment process, coordination of care and discharge planning needs. This ITP shall be reviewed and approved by an LPHA.

    2.      Revision/modification of the ITP as the client moves through the treatment process.

    Mental Health Therapy and Case Management

    1.      Provision of individual, group, couples/family counseling as driven by the needs identified in the assessment process and as indicated in the individualized treatment plan.

    2.      Provision of mental health case management to facilitate linkage to and ongoing coordination and support for needed services and supports in patients’ community environment.

     Spend at least 50% of work time providing direct billable services. Ten hours of direct mental health service per week and ten hours of direct mental health service per week are required.

      Other Duties: Performs tasks which are supportive in nature to the essential functions of the job, but may be altered or redesigned depending upon individual circumstances. 

    Standard Requirements:  Possesses a working knowledge of FGC applicable Standard Operating Procedures (SOP). 

    Knowledgeable of patient rights and ensures an atmosphere which allows for the privacy, dignity, and well-being of all patients in a safe, secure environment.

     Attend in-service and other training events to continue education and expand knowledge to improve the quality of patient services.

    Ability to follow written and oral instructions and procedures.


     

    Physical Demands: Ability to communicate in English via phone, in writing, and verbally in conversation with different levels of staff patient families, and any outside patients.

    2.      While performing the duties of this job, the employee is frequently required to stand and walk, often up and down stairs.  The employee is frequently required to sit; use hands or fingers, handle, or feel; reach with hands and arms, and talk or hear.  Specific vision abilities required by this job include close vision and ability to adjust focus.  Employee must be able to utilize a computer, phone, etc. for extended period of time and write or type notes for at least two (2) to four (4) hours per day.

    3.      While performing the duties of this job, the employee may be occasionally exposed to blood or other body fluids.  The noise level in the work environment is usually moderate.


    Schedule: Intern must be able to work three times a week, Monday through Friday, from 8:00am-4:30pm

    Experience and Skills:

    Education and/or Experience:  Master’s degree in Psychology, Social Work, Counseling, or a related discipline from an accredited institution preferred.  The Mental Health Therapist is a Qualified Mental Health Professional [QMHP] who has the education, skills and experience to conduct a clinical assessment, develop a plan of treatment and provide treatment for a person with mental illness, under the supervision of a Licensed Practitioner of the Healing Arts [LPHA].

     

    Certificates, Licenses, Registrations:   State of Illinois licensure required (i.e. Licensed Professional Counselor, Licensed Clinical Professional Counselor, Licensed Social Worker or Licensed Clinical Social Worker). In addition to the above licensure, IAODAPCA Certified Alcohol and Other Drugs Counselor certification or proven experience with substance use treatment preferred.
