Job Description:

Family Guidance Centers, Inc. (FGC), a not-for-profit behavioral healthcare organization, treats and prevents substance abuse, as well as an array of other related problems. **** $1000.00 dollar sign on bonus available for Mental Health Therapist, first 500.00 to be obtained after 30 days of employment, and 500.00 after 6 months of employment. !!!!******

   Essential Duties and Responsibilities are:

  • Completes SAMMS intake assessments for new patients.
  • Completes an individualized treatment plan for each patient on caseload, addressing all needs identified in the psychosocial assessment.
  • Provides group and individual sessions to patients in accordance with their treatment plan problems, goals, and objectives.
  • Properly documents all services provided in individualized progress notes per SUPR, Joint Commission, and FGC regulations, standards, and policies, summarizing the patient’s status and ongoing treatment needs.
  • Reviews and evaluates patients’ progress and continued stay in treatment in relation to measurable goals and objectives described in treatment and care plans. Modify treatment plans to comply with changes in patient status.
  • Plans and assists with the implementation of aftercare and continuing recovery plans once patients are discharged from treatment programs.
  • Works with patient and other staff, when necessary, to deescalate and assist with patient emergencies and crisis situations.
  • Coordinates care with other treatment providers, DCFS, probation/parole, and others as necessary, to help ensure patient needs are met and increase positive treatment outcomes.
  • Participates  in any treatment team staffings.
  • Makes appropriate patient referrals to outside agencies for the provision of ancillary services.
  • Maintains patient confidentiality at all times in accordance with 42 CFR and HIPAA regulations.
  • Develops a working knowledge of FGC applicable Standard Operating Procedures (SOP).
  • Knowledgeable of patient rights and ensures an atmosphere which allows for the privacy, dignity, and well-being of all patients in a safe, secure environment.
  • Assists with the collection of toxicology specimens, as needed.
  • Attends in-house and off-site trainings for continuing education, in order to expand knowledge, improve the quality of patient services, and to maintain licensure and/or certification at all times.
  • Utilizes calendar and scheduling software, email, and agency’s clinical software program. Experience using desktop computers, copier, and fax machine.
  • Other duties as assigned, including but not limited to performing tasks which are supportive in nature to the essential functions of the job, but may be altered or redesigned depending upon individual or programmatic circumstances.

Start date: ASAP

Schedule: Monday - Friday (6:15a - 2:45p) with Saturday rotation; some flexibility required to meet patient and agency needs.

Experience and Skills:

Education and/or Experience: Bachelor’s degree (or higher) and prior substance abuse treatment experience preferred. Will consider relevant internship experience in lieu of paid employment.

Certification, Licensure, Registrations: Licensure (LPC, LCPC, LSW, LCSW) preferred, and/or CADC certification required within two years of starting work as a counselor.
