Job Description:

Family Guidance Centers, Inc. (FGC), a not-for-profit behavioral healthcare organization, treats and prevents substance abuse, as well as an array of other related problems. Since 1969, FGC's comprehensive programming has educated and guided individuals toward their full potential as productive members within their own families and our society.

 ***$1000.00 dollar sign on bonus available, first 500.00 to be obtained after 30 days of employment, and 500.00 after 6 months of employment. !!!!***

  Essential Duties and Responsibilities:

  • Complete SAMMS intake assessments for new patients.
  • Complete an individualized treatment plan for each patient on caseload, addressing all needs identified in the psychosocial assessment.
  • Provide group and individual sessions to patients in accordance with their treatment plan problems, goals, and objectives.
  • Properly document all services provided in individualized progress notes per SUPR, Joint Commission, and FGC regulations, standards, and policies, summarizing the patient’s status and ongoing treatment needs.
  • Review and evaluate patients’ progress and continued stay in treatment in relation to measurable goals and objectives described in treatment and care plans. Modify treatment plans to comply with changes in patient status.
  • Plan and assist with the implementation of aftercare and continuing recovery plans once patients are discharged from treatment programs.
  • Work with patient and other staff, when necessary, to deescalate and assist with patient emergencies and crisis situations.
  • Coordinates care with legal, law enforcement, and treatment professionals, both within and outside the agency, to help ensure patient needs are met and increase positive treatment outcomes. This would include Will County representatives, Safe Passage program contacts, and others.
  • Participate in any treatment team staffings.
  • Make appropriate patient referrals to outside agencies for the provision of ancillary services.
  • Maintain patient confidentiality at all times in accordance with 42 CFR and HIPAA regulations.
  • Develop a working knowledge of FGC applicable Standard Operating Procedures (SOP).
  • Knowledgeable of patient rights and ensures an atmosphere which allows for the privacy, dignity, and well-being of all patients in a safe, secure environment.
  • Assist with the collection of toxicology specimens, as needed.
  • Attend in-house and off-site trainings for continuing education, in order to expand knowledge, improve the quality of patient services, and to maintain licensure and/or certification at all times.
  • Ability to use calendar and scheduling software, email, and agency’s clinical software program. Experience using desktop computers, copier, and fax machine.
  • Other duties as assigned, including but not limited to performing tasks which are supportive in nature to the essential functions of the job, but may be altered or redesigned depending upon individual or programmatic circumstances.


Schedule: Monday - Friday (9:00a - 5:30p), Group session (10:00a - 6:30p)

Experience and Skills:


Education and/or Experience: Bachelor’s degree (or higher) and prior substance abuse treatment experience preferred. Relevant internship experience will be considered in lieu of paid employment.


Certification, Licensure, Registrations: Licensure (LPC, LCPC, LSW, LCSW) or CADC certification preferred, and required within two years of starting work as a counselor.
