Job Description:

Family Guidance Centers, Inc. (FGC), a not-for-profit behavioral healthcare organization, treats and prevents substance abuse, as well as an array of other related problems. FGC is currently looking for a self-motivated, innovative hospital-based SOR Admissions Specialist for its Springfield location. Family Guidance Centers Inc. offers opportunities for individuals with an interested in behavioral health.

1st $500.00: after 30 days of employment

2nd $500.00: after 6 months of employment



Essential Duties:

  • Completes SAMMS intake assessments for new patients.
  • Completes an individualized treatment plan for each patient on caseload, addressing all needs identified in the psychosocial assessment.
  • Provides group and individual sessions to patients in accordance with their treatment plan problems, goals, and objectives.
  • Properly documents all services provided in individualized progress notes per DASA, Joint Commission, and FGC regulations, standards, and policies, summarizing the patient’s status and ongoing treatment needs.
  • Reviews and evaluates patients’ progress and continued stay in treatment in relation to measurable goals and objectives described in treatment and care plans. Modify treatment plans to comply with changes in patient status.
  • Plans and assists with the implementation of aftercare and continuing recovery plans once patients are discharged from treatment programs.
  • Works with patient and other staff, when necessary, to deescalate and assist with patient emergencies and crisis situations.
  • Coordinates care with legal, law enforcement, and treatment professionals, both within and outside the agency, to help ensure patient needs are met and increase positive treatment outcomes. This would include Will County representatives, Safe Passage program contacts, and others.
  • Participates in any treatment team staffings.
  • Makes appropriate patient referrals to outside agencies for the provision of ancillary services.
  • Maintains patient confidentiality at all times in accordance with 42 CFR and HIPAA regulations.
  • Develops a working knowledge of FGC applicable Standard Operating Procedures (SOP).
  • Knowledgeable of patient rights and ensures an atmosphere which allows for the privacy, dignity, and well-being of all patients in a safe, secure environment.
  • Assist with the collection of toxicology specimens, as needed.
  • Attend in-house and off-site trainings for continuing education, in order to expand knowledge, improve the quality of patient services, and to maintain licensure and/or certification at all times.
  • Ability to use calendar and scheduling software, email, and agency’s clinical software program. Experience using desktop computers, copier, and fax machine.
  • Other duties as assigned, including but not limited to performing tasks which are supportive in nature to the essential functions of the job, but may be altered or redesigned depending upon individual or programmatic circumstances.

 

 

Schedule: Monday - Friday (9:00a - 5:30p) with one night to 6:30p for group coverage

Experience and Skills:

Education and/or Experience: Bachelor’s degree (or higher) and prior substance abuse treatment experience preferred. Will consider relevant internship experience in lieu of paid employment.

Certification, Licensure, Registrations:  IAODAPCA Certified Alcohol and Other Drugs Counselor (CADC)  required or able to obtain within two (2) years of hire date Licensure (LPC, LCPC, LSW, LCSW) preferred.
