Job Description:

Family Guidance Centers, Inc. (FGC), a not-for-profit behavioral healthcare organization, treats and prevents substance abuse, as well as an array of other related problems. Since 1969, FGC's comprehensive programming has educated and guided individuals toward their full potential as productive members within their own families and our society.

Essential Duties:

  • Reviews assessed case management needs as expressed by clients and as indicated on the client treatment plan
  • Responsible for a minimum of 20 hours of production per week
  • Assists with monitoring patient engagement and communicates with the treatment team regarding patient attendance, participation, and any barriers or needs to support treatment involvement
  • Coordinates patient care with UI Health Primary Care Physician, Nurse Practitioner, FGC Nurse, FGC OMT Counselor and FGC Recovery Support Specialist
  • Assists with toxicology tests and entering accurate toxicology information into database
  • Communicates and assists all patients with their transportation needs which include regular contact with their insurance company and constant tracking and updating of all standing orders
  • Acts as liaison with the MCO designated transportation companies
  • Facilitates Orientation with patients to review FGC OMT handbook and tracks all newly admitted patients to ensure orientation is completed within 14 days of admission
  • Acts as back up staff member to assist in completion of the Intake Screening for Immediate Access to Care
  • Other duties as assigned

 

 

 

Schedule: Monday - Friday  (6:00a - 2:30p) with Saturday rotation

Education and/or Experience: An Associate Degree or Bachelor's degree preferred; A minimum of 1 year experience with substance abuse treatment.
