Job Description:

Family Guidance Centers, Inc. (FGC), a not-for-profit behavioral healthcare organization that treats and prevents substance abuse, as well as an array of other related problems, is currently seeking a Licensed Practical Nurse with some experience with drug treatment as well as clinical experience. Family Guidance Centers Inc. offers opportunities for individuals with an interest in behavioral health.

1st $500.00: after 30 days of employment

2nd $500.00: after 6 months of employment


Essential Duties:

  • Provides direct supervision to all staff nurses allocated to FGC at Wabash, which includes timekeeping approvals, commendations, disciplining, competencies, and performance evaluations.
  • Works (in person, via telephone, and via video conferencing) with the FGC Nurse Managers as needed regarding the nursing schedule.
  • Communicates with the HR department to coordinate and interview nurses seeking employment at the FGC Wabash clinic.
  • Provides direct training, education, and orientation to new staff allocated to FGC at Wabash clinic.
  • Works (in person, via telephone, and via video conferencing) directly with the FGC Nurse Managers to coordinate the FGC program physicians’ history and physical examination schedule for all assigned sites.
  • Infection control data collection and quarterly/annual reporting. Participates in the agencies’ performance improvement, infection control, and audit correction processes.
  • Responsible for the case review process for Code M’s, T’s, 1’s, and medication exception requests. Processes medical alerts using SAMMS software; documents all services performed in their appropriate service categories.
  • Responsible for completing the assigned clinics' Monthly Diversion Control reports.
  • Responsible for H&P and Vivitrol tracking for billing.
  • Processes patients from assessment to discharge for medication-assisted treatment, drug-free treatment programming, and assess patients for appropriateness for assigned clinics.
  • Responsible for follow-up with documentation for Pain Assessment and Nutritional Questionnaire patients.
  • Process involuntary withdrawal recommendations from AVP of OMT.
  • Responsible for completing Staff Nurse's initial competencies.
  • Able to assess patients for appropriateness for assigned clinics.
  • Properly administer and read alcohol analyzers.
  • Available via company-issued mobile phone to communicate with any appropriate FGC staff including nurses and managers after hours.
  • Administration of Vivitrol injections. Processes patients within all Vivitrol revenue funding sources.
  • Provides educational awareness to patients that Vivitrol is available as a third option in medication-assisted treatment.
  • Other duties as assigned.



Schedule: Monday - Friday  with rotating Saturdays

Experience and Skills:

Education and/or Experience: Two years of experience in a hospital or in-patient setting with 1-3 years of management experience  required. Basic nursing and substance abuse pathology knowledge. Strong working knowledge of state and federal rules and regulations and accreditation guidelines needed (SUPR, CSAT, SAMHSA & JC).


Certificates, Licenses, Registrations:  State of Illinois Licensed Practical Nurse (L.P.N.) and current certified CPR required .
