Job Description:

Family Guidance Centers, Inc. (FGC), a not-for-profit behavioral healthcare organization, treats and prevents substance abuse, as well as an array of other related problems. Since 1969, FGC's comprehensive programming has educated and guided individuals toward their full potential as productive members within their own families and our society.


Essential Duties:

  • Promotes a welcoming, positive customer service culture which encourages quality patient outcomes and promotes staff retention.
  • Develops referral relationships with our Shelter Partners, Homeless Outreach Teams, Task Force, County Hospital Social Workers, faith-based and local community organizations, serving as their liaison to accessing treatment services offered by Family Guidance Centers.
  • Strives to develop and direct prospective referrals to FGC’s Medication Assisted Recovery Mobile Van as it makes rounds in the neighborhoods of our community partners as well as surrounding county partners. Surrounding county partners include Cass, Menard, DeWitt, Macon, Macoupin and Shelby counties, which have been identified as MAR “deserts” by IDHS/SUPR in that they have virtually no access to one of the three FDA-approved forms of medication to treat opioid use disorder (OUD).
  • Assist with monitoring patient engagement and communicate with the treatment team regarding patient attendance, participation and any barriers or needs to support treatment involvement.
  • Coordinate Outreach workers schedule, follow up with all outreach visits, and ensure that all workers are fully engaging caseworkers and patients.
  • Confirm adequate outreach work is completed at all partnerships and in the field.
  • Oversee and support services delivered on FGC’s Mobile Van, including the completion of the data collection instrument.
  • Actively supports positive customer service, which encourages quality patient outcomes.
  • Document services provided in the individual patient record in a timely fashion and in accordance with IL DASA, Federal and FGC standards.
  • Demonstrate professionalism in working with patients, staff and other outside professionals or collaterals. Supervision and Professional Development Participate in supervision with AVP as well as appropriate continuing education activities related to care management and maintenance of relevant certification or licensure.
  • Describes FGC programs, services and eligibility requirements in laymen’s terms to people struggling with opioid and alcohol addiction and those at risk of overdosing, as well as family members or friends of people struggling with addiction, informing and assisting them to gain immediate access to treatment.
  • Communicates with FGC partnerships with hospitals, with the medication assisted recovery mobile van team, with FGC program managers at our MAR outpatient centers, and intake staff at all program locations, to coordinate access to treatment and broker transportation of community referrals to FGC service locations.
  • Maintains a daily work activity log that includes a listing of all prospective client names and contact phone numbers, referral status (i.e.: ‘in progress’ or ‘completed’), and communities visited.
  • Accurately completes the official FGC Community Intervention Billing log and submits on a weekly basis.



Work Schedule: Monday - Friday (8:00a - 4:30p) with flexibility to accommodate the agency's needs

Experience and Skills:

Education and/or  Experience:  Individuals with lived recovery experience who have a working knowledge of medication-assisted treatment and drug-free programming, hospital-based medical detox program admission and discharge policies and procedures preferred; Certification is Opioid Impacted/Behavioral Health Focus preferred

Certificates, Licenses, Registrations: Possess and maintain a valid Illinois Driver’s License and current vehicle insurance.
