Job Description:

Family Guidance Centers, Inc. (FGC), a not-for-profit behavioral healthcare organization, treats and prevents substance abuse, as well as an array of other related problems. Family Guidance Centers Inc. offers opportunities for individuals with an interested in behavioral health. 


Essential Duties:

  • Promotes a welcoming, positive customer service culture which encourages quality patient outcomes and promotes staff retention
  • Assists in performing Hospital Based SOR Warm Hand Off screening tools with patients in order to determine appropriate level of care and discharge planning needs
  • Conducts informational/educational sessions providing appropriate materials to educate patients on addiction recovery and aftercare options
  • Acts as coordinator for discharge needs of assigned patients
  • Maintains current knowledge of resources available within the community, maintains supply of resource materials to be distributed to patients when needed.
  • Networks with others to obtain other resources for patients as needed
  • Works collaboratively in formulating an appropriate and realistic discharge care plan, communicating with the patient
  • Completes appropriate forms, and contracts all appropriate individuals to facilitate the transfer/linkage of patients to other community-based agencies and services.  Ensures all referrals are linked through follow-up


Schedule: Monday - Friday (8:00a - 4:30p)

Experience and Skills:

Education and/or Experience: High School Diploma or GED  required; Associate degree (or higher) in Addiction Counseling, Psychology, Social Work, Counseling, or a closely related curriculum, from an accredited institution  encouraged to apply


Certificates, Licenses, Registrations: IAODAPCA Certified Alcohol and Other Drugs Counselor (CADC)  required or able to obtain within allotted time from hire date
