Essential Duties: Answers daily inquiries from patients and referral sources via phone and assists patients with placement in treatment services according to data obtained from screening based on ASAM criteria Assists in performing comprehensive psychosocial assessments with patients in order to determine appropriate level of care and placement when needed Make appropriate referrals for other community program services when necessary to assist patient Coordinate with Access to Care Coordinators and Access to Care Program Manager for immediate access to treatment services based on availability Prioritizes and organizes tasks according to the acuteness of situations as presented Treats patients with respect and dignity and maintains and respects the confidentiality of all patients in accordance with 42 CFR and HIPAA regulations Maintains good working relationships with FGC staff at other sites and assists with coordination of transfers Attends in-house and off-site trainings for continuing education, in order to expand knowledge, improve the quality of patient services, and to maintain licensure and/or certification at all times Other duties as assigned Work Schedule: Monday - Friday (8:30a - 5:00p) Start Date: ASAP Work Location: 120 N. 11th St, Springfield, IL Family Guidance Centers, Inc. is a Drug Free Environment (including THC and/or CBD) and an Equal Opportunity Employer.
